Evidence record

One record of everything recorded, signed and chained.

The Evidence record screen in Conditionary, shown with a modelled example organisation.
Shown with a modelled example organisation. Nothing on this screen is a real institution.

The evidence record is the ledger behind every other screen. Each entry says what happened, for which decision, recorded by whom, when, and with the details that evidence type asks for. Entries are signed by the person or system that recorded them and chained to the entry before, so a removed or altered line is detectable.

Filter by type, by person, by date or by individual case. Set a checkpoint to fix the record at a point in time. The record lives on your premises as ordinary files, and the verification runs on your machine.

On this screen you can

  • Record evidence directly, or from any screen that points to it.
  • Verify the whole chain in one click.
  • Set a checkpoint before an audit and show the record as it stood.
  • Filter down to a single case and see every entry about it.
